So I haven't blogged for awhile. And to update from my last entry even though its weeks late... YAY! I passed! I am already in my preceptorship. I was hoping to be in the hospital I was in from the last 2 rotations I had but unfortunately I ended up with the hospital I was in a year ago. I think that I would learn more if I went on the other hospital than this one.
My first day doesn't feel like my reorientation on the floor cuz my nurse threw me to the wolves per se. I was tired because it was my first day of doing a 12H shift.
2nd day of preceptorship was a lot of crap. This would be the 1st day of my nurse's 2-day shift. We don't know crap about our pt's except from what we gathered from the kardex. Beginning part of the shift things were late but running smoothly. We had 4 pts & a possible admit. Things started to go down after lunch when we had to discharge a patient then have to admit 2. Truthfully I don't know the hospital's admitting & d/c paperwork. I know the basic from class, but every facility have their own stuff. I wasn't oriented w/ those so I depend solely on my nurse to know & direct me w/ what I need to do. Crap! Then we had a patient who needs blood transfusion (which would run 3~4 hrs) & the pt have antibiotics/Iv piggybacks due during infusion time. I was like, oh so either run it bet infusion of the blood or start a new line on the other arm. My nurse waived the 2nd idea. Unfortunately that lead to more of our downfall that day. By the time the 1st unit of blood finished, run the 1st of the 2 IVPB's due it was also the end of the shift report. During the time that, that pt was receiving the blood, we had to d/c a pt (paperwork! paperwork! paperwork!) then admit 2 new pts (& more paperwork). Then one of the new pts was being a pain in the ass. Running back & forth for the care of the other pts, everything caught to us by the time we ended our shift so we had to extend our time. I have no idea how I drove home cuz I was sore all over. I haven't had the time to sit & rest my legs for 6H or so.
The next day (my precepting shifts depengs on my nurse's & my class schedule so I either have 1 or 2 straight days w/ him). I was still feeling sore but once I got to the flr, soreness was forgotten. I have an eerie feeling that d/t the census of the flr, we will possibly have an admit. We had 4 pts. Again, the 1st half of the shift went smoothly & all meds & morning care have been done. The snag we kinda got in the morning was that one of our pt's needed blood transfusion & the type & cross match for the antibodies wasn't done by the lab (& hell it was in the order!). The blood didn't get ready until in the afternoon.
All hell break lose in the afternoon...
- have to discharge 2 pts to SNF & at home = 3 d/c paperworks, charts to be closed, 2 hand off reports to the EMT's transferring the pt, 2 d/c teachings that's like about 30 mins or so per pt
- blood transfusion on the remaining pt was late d/t the lab mishap, that would run over 2H we got the blood like 2-3PM
- radiology people came & needs to pick remaining pt up for CT, unfortunately for them blood's still running so they can't take the pt
- after blood transfusion pt pooped *bangs head on the wall* & the radiology peeps came back to get her
- CNA being a pain & nowhere to be found (she was being a grouch all day cuz she's alone assisting 20 ppl. Lady, you choose this job. Rant to someone else)
- 2 pts came in
- need to do admitting paperwork (30 mins to an hour since they're direct admit @_@ dang so many paperwork...)
- then you have to wait for the admitting MD's order to initiate for the pt.
- need to do initial charting for both pts
- the new admit pts (can't blame them) were hungry & were asking for food... have to wait dunno how many minutes after we ordered food for them
- procedures to be done with the new admit pts
- IV line needed to be started with 1 of the new admit. Nurse fail to start. I would start it if I only knew. I was running around helping other pts (I don't think my nurse can start IV d/t his gout in his hand. & I have no idea why he can't admit it to the others. Might he lose his job over it?)
- by the time pt was done w/ everything we need to hang the 2nd unit of blood, but it was already an hour before the night shift starts. no time to grab the blood & hang it because we need to close out notes
all those things happened bet the span of 4-5H, sounds easy to finish w/in that time alotted but there are other factors to consider
by the time the MD was finished with the orders it was already 4PM, the secretary didn't finish encoding into the system until 430pm (can't blame her, we have 8 admits on our side of the flr, nurses & MD's everywhere) something wrong with the system that we have to wait 1.5H for the new pts to appear in the system so we can't give the scheduled meds. everything's in the pixys >.> & unfortnately I have no access to it. You can guess it's already 6PM, our shift ends at 7PM. Night shift nurses started to arrive & ask for report.
I can't really say anything about the night shift, but from what I have observed in the past few days, they don't get that much admit, they don't give that much meds at night, (I mean 1-3 pills +/- insulin coverage & 1-2x abx VS morning care, mornining meds that are 2-3 pages long, most of the abx are given in the AM, most coverage are done in the AM, most procedures come in the AM, most MD's come in the AM give orders so we have to write those down, once labs come in they want us to call them, & the patients are AWAKE at this time x.x, so that call light woudn't stop flashing!), things are much calmer at night AND THEY COMPLAIN TO US!
Stop sitting around & making chismis...
Super multi-tasking in the morning & we get grouchy night shift nurses. That arrogant night shift nurse should experience doing a morning shift like that & let's see if she can handle it by herself.
*breathes* For 2 days tempers were rising & I'm trying to be aloof hahaha! I don't want to add to the tension.
Anyway, my nurse should be thankful because there's a student w/ him doing half of his work. (And I can assure you that I am being safe with my patients.) I'm doing half of the paperwork that I can accomplish like charting for the patients, doing the routine care, completing his hands-off communicationsgiving the meds & answering the pt's demands. It's sad that when he complains he adds the fact that there's a student with him that he's teaching. Truth to be told I'm not learning anything new except the experience of being on the flr & being the nurse to the pts. :/ I can't complain cuz he's nice to me in a way (I just don't know what he tells me behind my back)
UGH! When I got back home I had twice the body pains then I didn't notice that I fell asleep on the couch ]= My mudra had to wake me up & I was like, I fell asleep? *cry*
Anyway time for me to sleep now since I had a lot to do tomorrow morning... Yasumi *_*
